Dr Parameshwara C.M., MBBS, MS, FIAS, FISCP, MBA (Health Care), Associate Professor of Surgery and Consultant Colo Rectal Surgeon International Institute of Coloproctology (IICP), is doing pioneering work on par with any international single speciality facility. While many professionals would shun the Colo Rectal sphere, he specifically chose to pursue this path as he saw immense suffering and pain in patients suffering from Colo Rectal problems during his practice in Mandya. In typical rags to riches story, he has metamorphosed from the throes of abject poverty to owning one of the best Colo Rectal Institutes in India, which is on par with international facilities. Significantly Dr.Parameshwara and his dedicated staff follow the philosophy of providing the best Colo Rectal health care practices to all patients at affordable costs, and sometimes even treating patients free of cost. He was born on September 13, 1971 in Kolar District and his father K.H. Munikrishna migrated to Bengaluru in search of a job as he was a landless labourer.
SMILES GASTROENTEROLOGY is associated with Agency for Medical Innovation, Austria to impart training programs for surgeons on Colo Rectal issues. he training program, one of the best in the Asia Pacific region, has so far trained 36 doctors from across India. he training program scheauled for April is expected to attract foreign participants too. When his father knocked on his doors, he went out of the way to get him a job albeit as a cleaner of buses. Unfortunately he could never get any promotion and retired as a bus cleaner as he was illiterate and not willing to upgrade his knowledge. However working in a prestigious public sector had its advantages as his children, including Dr. Parameshwar and two daughters could get good schooling education in BEL School and PU College. Dr. Parameshwar had inherent academic skills of working hard along with a passion for extracurricular activities, he participated in the NCC cadet and was a Senior Under Officer as a PUC student to develop leadership qualities early in life. He is grateful to his maternal uncle and grandfather late Nanjegowda for helping him in his education and fulfilling his needs.
Though his father aspired that he take up apprenticeship in Bharat Electronics those days and settled down to a public sector life, he was adamant to continue his PUC education for the sheer reason he had made good friends. His decision paid dividends as he topped his college in PUC and cracked a seat in Bangalore Medical College (BMC) for his MBBS with a CET ranking of 151 in 1989. In pursuit of further excellence in the field of health care, he cracked the postgraduate CET exam with a rank of 19 and opted to pursue his MS in General Surgery again from BMC from 1997 to 2000. He, however, aspired to become a civil servant as he had as he came under the influence of political leaders of his time as president of the Karnataka State Junior Doctors Association for two years when studying for his MS. Despite spending three years from 2001 to 2003 in New Delhi and working hard, he failed to crack the Indian Administrative Services’ entrance examination.
He was married to Mandya political leader and former minister S.D. Jayaram’s daughter Anuradha by then and the three years in New Delhi had literally emptied his pockets. Desperate to earn a living and stand on his own feet, he abandoned hopes of administrative services and returned home in preparation for jobs in UK. It was then that a relative from Mandya suggested that he could set up practice in his wife’s home town by establishing a hospital. That was the biggest turning point of his professional life as he set up his first 120-bedded hospital along with a Nursing School in Mandya in 2006 and also joined Mandya Medical College as faculty. He worked for 14 years, mentored over 1,400 students in surgery skills and recently resigned as Associate Professor of Surgery. In 2008, he started an eye hospital in Mandya and followed it up with another 50-bedded hospital in 2009. In 2011 he launched a Paramedical College in Mandya and in 2015 started a unique Inventure Ashoka Shaala – a school for students from LKG to eighth standard.
During his service in Mandya, he came across many women suffering from Colo Rectal issues and ignorant of whom to approach for treatment. Most often they would end up being treated by quacks who could cause them more damage and life-long embarrassment. Around the same time a doctor who was his junior was traumatised after getting treated by a quack and was on the verge of committing suicide. Dr. Parameshwar took him under his care, gave his shelter in Mandya and treated him successfully. Thus mulling the options of setting up a single superspeciality facility he conceptualised SMILES (Silicon Valley Minimally Invasive Laser and Endoscopic Surgery) in 2011 in Mandya. The concept got structured in 2014 with the launch of state-of-the-art facilities in Bengaluru as well as Mandya and in now on the cusp of greater achievements with the incorporation of SMILES International Institute of Colo Proctology (SIICP) in 2017.
In order to learn the best practices in this unique health care segment, which is generally an unchartered territory, he travelled extensively in India and abroad including Germany and the US. He also completed two Fellowships and in the last five years has honed his skills to become a competent Colo Rectal Surgeon. He has also attended many national and international conferences where he has presented a number of papers. From 2014 till date he has completed 14,000 procedures, and prior to this from 2005 to 2014, he has performed over 10,000 surgeries. The facilities in the institute included the best of equipment from across the global including the US, Italy and advanced laser machines. The institute treats patients from 15 foreign countries including Canada, US, UK, UAE, Bangladesh.
Inspired by his mentor and management education guru D.V.R. Seshadri of Indian School of Business, Hyderabad, the institution works on the simple philosophy of “never return a patient untreated even if he doesn’t have money”.
Even the poorest of the poor are treated with the same care as those patients who pay for the procedures. This is possible because the institution has partnered with a consortium called Equitable Health Care Consortium founded by legends G.N. Rao, Chairman of LV Prasad Institute, Hyderabad, Arvind A.K. Madurai and Indian School of Business Hyderabad. Dr. Parameshwar is also a found member of the foundation. In order to create awareness on Colo Rectal health issues, SMILES Gastroenterology has partnered with Dhan Foundation, the biggest NGO in the world founded by Vasimalai an alumnus of IIM Ahmedabad. The institution has covered 26 lakh families in six districts, including Ramnagara, Mandya, Chamarajnagar, Mysuru, Hassan and Tumakuru on a door-to-door campaign creating awareness on Colo Rectal problems, lifestyle diseases, good lifestyle practices, colon cancer, open defecation; pregnancy and women problems as well as awareness on approaching quacks. In the future, the institution plans to cover other districts of the state and gradually the whole country.
This area of specialisation is the most neglected even in medical education as it involves “dirty work” with stigma attached to it. There is a lot of misconception and when patients go to doctors with piles, fissure or fistula, they just operate the ailment without going deep into the causes. Piles, fissure or fistula is a window to the much bigger problem in the GI (gastrointestinal) tract. Bleeding in the rectum, loss of weight, loss of appetite; altered bowels, constipation or diarrhoea may be symptoms of colon cancer too. In the genes His father K.H. Munikrishna aged 81 and mother Menakamma aged 73 are still physically active. Dr. Parameshwar has two sisters; one sister has done her MTech from Bengaluru and works in a senior position in a multinational financial services company and the other is a teacher in a well-known school in Bengaluru. He is married to Anuradha S.J., daughter of former minister late S.D. Jayaram of Mandya. She is a homemaker but helps in the administrative work of the hospital. Dr. Parameshwar and Anuradha are blessed with a son Manitkrishna Jayaram, who is studying in Ramakrishna Vidyashala, Mysuru, and daughter Ridhi, who is studying in National Public School, Bengaluru.
In a unique treatment protocol, the institute is associated with Hitam Ayurveda of Hyderabad and Banaras Hindu University to integrate Ayurveda and Allopathy treatments – a first of its kind in the world.
